What To Expect On Your First Visit

For your first visit, please wear loose-fitting, comfortable clothing that will allow the physical therapist to evaluate the injured area. If you have a problem involving your foot or ankle, please bring the footwear that you normally wear.

Bring with you a list of any prescribed medicines you are currently taking as well as over the counter medications you take regularly.

Please download and fill out the patient forms on this website in advance to expedite the paperwork when you arrive. These links are available on the bottom of this page.

Your initial evaluation will last about 60 to 75 minutes. You can expect to have a Human Performance Test done at this time. Follow-up visits average between 45 and 60 minutes, typically two to three times a week, depending on your doctor’s physical therapy prescription and your type of condition or injury.

Insurance

Med Center Therapy accepts Medicare patients, PIP, Worker’s Compensation, most group health insurance plans and self-pay with a prescription.

We are happy to check eligibility and benefits for you. Please provide us with your insurance company name, group number, insured’s identification number, date of birth and insurance phone number. We also provide insurance billing services for our clients.

Co-payments are collected at each visit as are deductible amounts that exceed $500.
